Legal Name Change Notification

    Individuals who have legally changed names can request to update their official name of record with Rockhurst University.

     

    Students can bring legal documentation of name change to the Registrar's Office in Massman Hall along with proof of photo ID (driver's license, passport, etc.).


    Students who cannot come to the office in person, see below:

    IDENTIFYING INFORMATION
    BIRTHDATE
    NEW / CURRENT LEGAL NAME
    For Active Students Only: If you are an active, enrolled student and wish to also update your name on your Computer Services accounts (student email address, Canvas classroom records, etc.).
    INSTRUCTIONS

    To complete this request, you must provide documentation through one of the following means:

    1.  Fax a notarized copy of legal documentation, copy of photo ID, and a copy of the confirmation email (after you submit your request) to:  816-501-4677.  This fax comes directly to the Registrar's Office.

    2.  Mail notarized copy of legal documentation, copy of photo ID, and a copy of the confirmation email (after you submit your request) to:

    Any documentation submitted via mail or fax that is NOT notarized, will not be accepted.  We will also not accept documentation submitted via email, as it is not considered a secure method of communication.
    NOTE - the completion of this form only indicates your INTENTION to make a formal change of name with the University. No official name change can be completed by the University until official documentation is received in the Registrar's Office.
