Consent to Conduct Business Electronically

I consent to conduct business electronically with Rockhurst University regarding my participation in the Dual Credit Program. I understand that the University may provide information and communications electronically, including but not limited to:

Course registration and enrollment
Class schedules and grades
Academic records and transcripts
FERPA-related communications
Tuition billing and payment notifications
Other notices related to my participation in the Dual Credit Program

I understand that Rockhurst University will send email notifications to the email address(es) listed in my application when my payment information is available. It is my responsibility to monitor my email and submit payment by the stated due date.

Student Financial Responsibility Agreement

Payment of Fees/ Promise to Pay
I accept responsibility for payment by the due date of all Rockhurst University charges assessed to my student account for participation in the Dual Credit program, including tuition and applicable fees. I understand that administrative, clerical, or technical billing errors do not relieve me of my responsibility to pay the correct amount owed.

Past-Due Accounts and Unenrollment

I understand that there is a set payment deadline each semester for all Dual Credit charges. The payment deadline will be published in the annual Student Key Dates & Deadlines document and communicated directly to students via email.

Following submission of the Dual Credit application, I will receive an initial payment email with instructions for making my payment. I understand that payment reminders will be sent every 10 days until the payment deadline or until my payment has been received.

I understand that if my account remains unpaid after the established payment deadline, I may be unenrolled from the Dual Credit Program on the Official Unenrollment Date. I understand that this unenrollment is final.

All returned checks are subject to a return fee. An additional fee may be charged if an electronic payment is returned unpaid due to insufficient funds, a closed account, an invalid account number, a stop payment, or the inability to locate the bank account.

IRS Form 1098-T

I understand that Rockhurst University may request my Social Security Number (SSN) or Taxpayer Identification Number (TIN) as required by Internal Revenue Service (IRS) regulations for Form 1098-T reporting purposes.

I consent to receive my annual IRS Form 1098-T electronically. I understand that, if I do not consent to electronic delivery, a paper copy will be provided upon request. I understand that I may withdraw this consent or request a paper copy by following the instructions provided by Rockhurst University.
